Salvation Army Christmas Sign Up

If you are in need of Christmas assistance, you are welcome to visit the Park Place Community in Room 13.  You must bring the following items to complete an application:

  • Picture ID for Head of Household AND Proof of Address.
  • Approval Letter from FSSA Office or Food Stamp/Medicaid Case Number.
  •  (If household does not receive one of these, then you will need to provide proof of income and expenses for the last 30 days)
  •  Social Security Card, Birth Certificate OR Medicaid Card for ALL Household Members.
